The following process has been setup in order to be in compliance with the latest mandate by the Governor of Illinois and the Chicago Archdiocese, that all school employees must be vaccinated or submit to regular Covid testing.   


Faculty and Staff will take a picture or scan of the back and the front of their Covid vaccination card. They will then email both photos to covidcompliance@carmelhs.org. This email address will be monitored by a approved group, in order to be in compliance with all medical regulations. 


  1. Vaccination card receipt
    1. Emails that are sent in will be received by everyone in the group in there Carmel email inbox.
    2. Download file and save to Google shared folder Covid Compliance
    3. From your Gmail account, open email.
    4. Hover over the first image so the save options appear and click the middle icon to Add to Drive.
    5. The middle icon then will change to a folder to Organize in Drive, click on this folder icon.
    6. Now click on the Move this item.
    7. From the next screen navigate to Shared drives > Covid Compliance > click Move Here.
    8. Reply to the sender, that there information has been received, and delete email.
  2. Vaccination card upload
    1. In Axiom from the Main page go to Find Staff/Faculty.
    2. Search for the person who's card you would like to upload by using the Full Name field and the Run Query. 
    3. From the results screen click on the persons name.
    4. From the person file we will add a file for both the front and back picture of the card. Click on Files under Other from the left side menu.
    5.  Click on Attach Files and a new window will open.
    6. From the new window, click on browse files, and a Open dialog box will open.
    7. Browse to the file by click on Google Drive from the left menu > Shared drives > Covid Compliance > choose file you want to upload by clicking on it and then click Open
    8. From the upload type chose COVID Compliance.
    9. Click + Add more, in the upper right hand corner.
    10. Repeat steps 7-9 above to add the other side of the card.
    11. Click Upload 2 Files. 
    12. Click Done, and close file attachment window.
    13. Go back to your window with the person record, and refresh, and you will now see the files attached.
  3. Vaccination card tracking.
    1. From the person record, click the lighting bolt icon and scroll down to Action in the pop up menu and choose Add Event...
    2.  In the event window enter the following information Person, Event Type (Covid Compliance), Date (todays date), Send Email (No), and Internal Only (Yes).
    3. Click Update to save.



To review who does not have a valid Covid Card on file run this query. It will display a list of Faculty/Staff that do not have a Individual Event attached for Covid Compliance. This query will provide a list of volunteers, coaches, etc.
