Upload a Health Plan to a student’s record

  1. Run a “Find Students” query and open the student’s record

  2. Click on the “Files” tab

  3. Click on “Attach Files”

  4. Click on “Browse” and choose the plan file you want to upload

  5. In the drop down box, choose the type “Medical Action Plan”

  6. Optional: Add any notes you want to go along with the plan in the blank box next to the type drop down.  These notes will be viewable by other users.

  7. Click “Upload”


Uploading plans for teacher view (non-axiom users)


When you upload a plan document for a student, there is a second location that you have to upload it to in order for non-axiom users (teachers) to be able to view it as well.  


2nd Step of uploading a plan

  1. Click on the Events Tab in the left hand column

  2. Click the green Add Record button

  3. Choose the date 

  4. In the Student Event column, scroll down to the bottom to choose HLTHDOCS:Medical Action Plan

  5. Click the green Update button

  6. Once it is added (it will most likely jump up to the top of the list because they are in "most recent" order), click on the pop out icon next to the plan

  7. Click on the Files tab in the left hand column

  8. Click on Attach Files

  9. Click on Browse

  10. Choose the file to upload (this is the same document you uploaded in the Files tab initially)

  11. In the drop down box, choose the type “Medical Action Plan”

  12. Click Upload & Save

  13. Close this tab.  You will have to refresh the tab you were working in (Individual Event tab) to see that the file has been attached.


To signify to teachers that a student has a medical plan

  • Add a note in the Medical Alert Field of their record.  
  • This will add a icon in the teachers gradebooks.

Additional note:

Anything listed in the medical alert field will be listed when the teacher hovers over the red heart icon in their gradebook.  If you also want the teacher to know that the student has a Medical Action Plan when they hover over the red heart, you can add “This student has a Medical Health Plan” to the alert field.  

In order to quickly find students who have a medical health plan, after you upload them, you can run this query and edit their Medical Alert field right here in the query by double clicking in the field. You can update multiple alerts at once before hitting Update.

Teachers will have a button in their portal that shows them the uploaded Medical Action Plans for any students who they have in their classes (Located in the same place they can see Plan Documents in their portal)

To Remove a Plan from a Student and Remove the Associated Icon from Teacher View

  1. Open the query above that you used to add the General Alert: https://axiom.veracross.com/carmelhs/#/results/423407 

  2. Delete the plan wording in the General Alert field of the student whose plan you want to remove, and click “Update”.

  3. Click on the student’s name to open his/her record.

  4. Go to the Events tab, find the plan you want to delete, and click the red “X” on that row.  Click “Update”.

  5. Go to the Files tab, find the plan file you want to delete, and click the red “X” on that row.  Click “Update”.
